The Initiator Registration List should be printed and prepared by the initiator according to the prescribed format.
By signing or affixing a seal on the Initiator Registration List, the initiator is deemed to declare that they do not fall under the disqualifying conditions specified in Paragraph 2, Article 8 of the People’s Organization Act, and accept full legal responsibility for the declaration.
